[SchoolFinance] 403B/TPA/Plan Documents?

We are looking at the process of offering a 403B/457B to staff.

For those that already offer a 403B/457B which provider did you use to
create plan documents? Do you use the same provider as the Third Party
Administrator? Can you provide a rough cost estimate of what creating the
plan was and the ongoing yearly costs?

Would anyone be willing to share their "Plan document" as an example?
